Book Dedication
Book dedications are a great way to honor a teacher, celebrate a birthday, or mark a milestone. It is also a wonderful way to continue to build our library collection.
Not sure which book to purchase? Please feel free to contact Mrs. Hiriak, our school librarian, at MHIRIAK@cbsd.org for a topic, an author, or a type of book.
To dedicate a book, fill out the form below and send it to the Mill Creek Library: Attn: Mrs. Venugopal. The cost is $15 per book, which is the average cost of a hardcover book.
Also, the library is currently trying to build an audiobook collection. If you are willing to dedicate an audiobook, the cost would be $25 per audiobook.
A personalized book label will be placed in the book or audiobook, and the person to whom the book is dedicated will enjoy being the first one to check it out of the library. All dedications will be announced on the morning announcements. Your support of this worthwhile program is appreciated! If you have any questions, please call extension 3605 for the library.

Please return this form in an envelope marked “Mill Creek Library: Book Dedications” with a check for $15 made payable to “Mill Creek Elementary.” The ordering and delivery process can take 4 to 6 weeks.
